If your debit card has been lost or stolen, please do not complete this form.  Call 800.947.5328 to report a lost or stolen card and receive a new debit card.

      Cards may only be issued to joint owners on your Share Draft Checking account.

    • If you select "Yes," we will apply the same overdraft protection to your debit card as currently protects the checking account it will access.
       

      If funds are not available in your overdraft protection account(s), do you want your debit card to access your available Privilege Pay limit? (Please click here for more information.)



    • In approximately two weeks your new debit card will arrive in the mail. You'll receive a randomly generated PIN in a separate mailer. You may change your PIN to a number of your own choosing at any CU Anytime ATM or at any SLFCU branch.

      Your debit card can be used in ATMs and at merchants worldwide. If you have an SLFCU CU Anytime ATM card, your ATM card will be deactivated after you've had your new debit card for 30 days.
